FSMTB Education Verification Request Form

Effective July 1, 2017, the FSMTB requires substantiation documentation to be sent directly from schools to FSMTB. For current students, this is included in the cost of tuition and will be uploaded after class #70, provided the student is not on academic or financial probation. There is a $10 fee to process FSMTB education verification requests for alumni.

Financial requirements for FSMTB Education Verification (alumni only)

  • Paid $10
  • Paid all non-tuition fees

Tuition paid in full OR good standing with TFC Payment Plan (see Payment Options)

Records processing is $10 per program.  Each records request is considered a separate processing fee.

Processing of the Records Request Form(s) will not begin until paid in full. Processing and shipping takes between 7-10 business days.
