Was a PTS cancelled? Please let us know!
If a student or an instructor cancels his or her PTS, instructors are required to fill out this form to notify the faculty coordinator of the cancellation.
Please remember, it is the responsibility of whoever cancelled the PTS to work with the student/instructor and faculty coordinator to reschedule. For example, if the instructor cancels the PTS, we appreciate you working with the student and faculty coordinator to reschedule the session.
